Frequently Asked Questions - Long-Term Disability
Long Term disability provides a benefit equal
to two-thirds of the employee’s regular monthly salary for accidental
bodily injury, disease, or illness. A benefit equal to 100% of
the employee’s regular monthly salary is provided for a physical
disability due to a line-of-duty injury, if caused by outside
force or violence.

is eligible to enroll in the Long Term Disability Program?
The long Term Disability Program is available
to permanent, full-time employees of employers participating
in the Utah Retirement Systems and to regular full time employees
who are covered by a retirement program offered by TIAA,
who elect to cover their employees under the program.

Do
I need to complete an enrollment form?
If you are an eligible employee, and your employer
participates in the Utah Retirement Systems or TIAA and has
elected to cover their eligible employees, your enrollment
is automatic, you do not need to complete an enrollment form.

Are
there any pre-existing waiting periods and/or medical conditions?
A pre-existing medical condition is a medical
condition, injury, illness, disease which was diagnosed, treated,
or for which signs or symptoms (known or unknown) occurred prior
to the employee’s coverage date.
The pre-existing waiting period begins the
date your coverage began and is in effect for one year of continuous
enrollment in the Program.

If
I become disabled, when will I start receiving a monthly income?
There is a three month elimination period
at the beginning of each period of total disability for which
NO BENEFITS WILL BE PAID. The elimination period begins
with the date of disability.

Are
the benefits I receive from the Long Term Disability Program taxable?
Yes, Long Term Disability benefits are subject to state and federal income taxes.

How
many months will I receive disability payments?
Eligible employees may be approved for benefits
for up to 24 months if they are totally disabled from their
regular occupation. Benefits beyond the initial 24 months can
only be approved if the employee is totally
disabled from any gainful occupation, if caused solely by physical
impairment.

What
information is required to start receiving benefits?
The first step: Call PEHP’s Long Term
Disability Program. An interview with the department will be
made. Interviews can be by phone or in person.
The second step: You must complete
a detailed application/claim/medical release form and return
it to Public Employees Health Program’s Long Term Disability
Department.
The third step: PEHP’s Long Term Disability
Program will then request detailed information from medical
providers and your employer

Can
I still accrue retirement service credit while I am disabled?
If your employer has selected PEHP’s
LTD Plan, retirement credit with the Utah Retirement Systems
will accrue during the period of disability, unless the disabled
employee is exempted from the Utah Retirement System.

Rehabilitation is an important part of the Program.
All employees applying for or receiving benefits under the Program
will be evaluated for potential rehabilitation services. Rehabilitation
efforts are most effective when they are started as soon as possible.
Rehabilitation includes physical and psychological benefits.
All benefits received from the Long Term Disability
Program are coordinated with other income, such as Social Security,
Worker’s Compensation or other disability plans or pensions.
