An Electronic Trading Partner Agreement (ETPA) must be executed to permit access to the above information. If you have not signed an agreement, please print the agreement and fax the entire document to your Provider Relations Representative.

Does your office staff need training on any of PEHP’s policies, procedures, and/or benefits? We would be happy to visit your office. Please call your Provider Relations Representative to schedule an appointment.

NPI (National Provider Identifier)

The NPI is a unique health identifier for physicians, health care professionals, dentists, and facilities. This assigned NPI will not expire and will replace all the provider identifiers used today in standard health and dental care transactions for all carriers.  All providers are required to obtain and use the NPI by May 23, 2007.

PEHP encourages all providers to apply for their NPI as soon as possible.  Do not wait until 2007.  PEHP will begin testing electronic transactions with providers who have obtained their NPI in spring/summer 2006.

Please contact your PEHP Provider Relations Representative as soon as you have obtained your NPI.

Providers may apply for their NPI in one of two ways:

  • Through an easy web-based application process at https://nppes.cms.hhs.gov.
  • By submitting a paper application available either on the above web site or by calling Fox Systems, Inc. at 800-465-3203.
