This interactive map helps you find low-cost hospital alternatives for common medical procedures. Use this map when you need a colonoscopy, surgery, MRI/CT scan, and other treatments.

Traditional Plan
(Automatic - No application needed)

  • Pay no more than a $45 copay on all services when you choose the providers on the map. No deductible, no hidden fees.*
  • Always earn cash back for certain preventive services (e.g., colonoscopy) at select locations.
  • After reaching your Out-of-Pocket Maximum, you can earn cash back on additional services.

High-Deductible Plan (e.g. STAR HSA Plan)

  • Earn cash back when you visit providers on the map. Once you’ve met your deductible, pay no more than a $45 copay.*
  • After your Out-of-Pocket Maximum, you can earn additional cash back.
