• Log in to your Express Scripts Personal Account.
• If you have an eligible prescription, you can transfer it to home delivery from your local pharmacy. On your Express Scripts Personal Account, you’ll see any eligible medication under “Transfer to Home Delivery.” Simply add the medication to your cart and complete checkout to switch to home delivery.
• Your doctor’s office can also send your prescription to Express Scripts Home Delivery electronically from their office or by fax.
• If you have a paper prescription from your doctor, fill out an Express Scripts Home Delivery order form and send it along with your prescription and your payment option to the address listed on the form. Your medication will be delivered within 14 days after Express Scripts receives your order.
