Reminders
- To help prevent delays, please avoid sending X‑rays, narratives, periodontal charts, or other documents unless they’re specifically requested by CDT guidelines. Please refer to the CDT Dental Code Guide for specific billing requirements. In general, claim attachments are not needed unless the CDT Dental Code Guide indicates they’re required.
- All electronically submitted claims must include the provider’s taxonomy code to be processed correctly. Claims missing a taxonomy code will be denied. Learn More.
- You can access the Fee Schedule Lookup tool in the Provider Portal under either Quick Access or the Resources menu.
- Preauthorization is required for certain dental procedures. Learn More.
